Blond Psyllium and Carbidopa

Here is the interaction we found between these two, with the evidence behind it.

Minor

ORAL DRUGS

Why this applies to Carbidopa

Carbidopa falls under ORAL DRUGS — the group Blond Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: blond psyllium is a soluble fiber that swells into a gel in your gut. That gel can grab onto things you swallow, including pills, and slightly change how much medicine gets absorbed. With carbidopa, the concern is mainly about timing, not a dangerous chemical reaction. Carbidopa works in the gut and bloodstream to protect levodopa, so if psyllium blunts its absorption, your Parkinson's medicine could feel a little less reliable (more stiffness, slowness, or tremor breaking through).

The good news is the evidence here is weak and theoretical for this exact drug. An easy fix is to separate them, taking psyllium about 30 to 60 minutes after your carbidopa dose. If you notice any change in how your symptoms are controlled, check with your pharmacist or doctor.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Carbidopa page

“Carbidopa (Lodosyn) keeps levodopa working in Parkinson's disease, so anything that blocks its absorption can cost real motor control, and two of the 21 supplements on this list do exactly that at a level rated major. Magnesium is the one that surprises people: in a human study it cut carbidopa absorption by 81 percent and levodopa's b…”

About Carbidopa

What Is Carbidopa Used For?

Brand name: Lodosyn

Carbidopa tablets — sold under the brand name Lodosyn and as generic carbidopa — are used together with levodopa or carbidopa-levodopa to manage symptoms of Parkinson's disease, postencephalitic parkinsonism, and symptomatic parkinsonism that can result from carbon monoxide or manganese poisoning. Carbidopa is never used alone; it always works as a partner to levodopa-based therapy.

Lodosyn is specifically used when a patient taking carbidopa-levodopa isn't getting enough daily carbidopa from that combination, or when a patient needs the two drugs individually titrated (adjusted) rather than taken as a fixed combination. Adding carbidopa helps reduce the nausea and vomiting that levodopa often causes and allows lower doses of levodopa to be used.
