Why this applies to Hydromorphone

Hydromorphone falls under ORAL DRUGS — the group Blond Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.

Here's the simple version: psyllium is a gel-forming fiber, and when you take it at the same time as a pill, that gel can sometimes grab onto the medicine in your gut and change how much gets absorbed. With hydromorphone, the concern is mostly theoretical, and the most likely direction would be slightly less drug getting into your system if you swallow them together. Hydromorphone works as-is (it isn't a prodrug), so reduced absorption could mean a bit weaker pain relief.

The good news is this is rated minor and the evidence is limited. An easy fix is timing: take your hydromorphone first, then your psyllium about 30 to 60 minutes later, with plenty of water. Since hydromorphone is a strong opioid, please run any changes past your pharmacist or doctor.

Blond psyllium is a soluble fiber from the seed husks of Plantago ovata, and it is one of the better-studied fiber supplements. It can help with both constipation and diarrhea and may modestly lower cholesterol and blood sugar. It is generally well tolerated when taken with plenty of water, but always start low and check with your pharmacist or doctor first.

What Is Hydromorphone Used For?

Brand names: Dilaudid, Hydromorph Contin, Palladone

Hydromorphone is used to manage pain severe enough to require an opioid analgesic — when non-opioid medicines or other options have not worked or are not expected to work well enough. It is available in several forms: oral tablets, oral solution, rectal suppositories, and injectable formulations given into a vein, muscle, or under the skin.

Hydromorphone hydrochloride extended-release tablets are specifically for severe, persistent pain that requires around-the-clock opioid treatment for an extended period, and are only for opioid-tolerant patients — they are not for as-needed use. The high-potency injectable formulation is also reserved for opioid-tolerant patients who need higher opioid doses.
