Blond Psyllium and Mexiletine

Here is the interaction we found between these two, with the evidence behind it.

Minor

ORAL DRUGS

Why this applies to Mexiletine

Mexiletine falls under ORAL DRUGS — the group Blond Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the situation with mexiletine and blond psyllium. Psyllium is a thick, gel-forming fiber, and once it swells up in your gut it can grab onto medications you swallow around the same time. That may slow down or slightly reduce how much of a pill gets absorbed. We don't have specific studies on mexiletine itself, so this is mostly theoretical and rated as a minor concern.

Because mexiletine is the active drug (not a prodrug), the worry would be getting a bit less of it, which could make it work less reliably for steadying your heart rhythm. Since this is a heart-rhythm medicine, consistency matters. An easy fix: take your mexiletine first, then take psyllium about 30 to 60 minutes later. Please run this by your pharmacist or doctor before changing anything.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Mexiletine page

“Mexiletine (Mexitil) steadies the heart's rhythm, and its benefit depends on holding a fairly precise blood level, so the 152 supplements on this list deserve real attention rather than a shrug. Only one entry, activated charcoal, is rated major, because it can soak up the drug in the gut and cut how much actually reaches your bloodstr…”

About Mexiletine

What Is Mexiletine Used For?

Brand name: Mexitil

Mexiletine Hydrochloride capsules are indicated for the treatment of documented, life-threatening ventricular arrhythmias — for example, sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers). Because mexiletine itself can trigger arrhythmias, it is not recommended for minor or symptom-free irregular heartbeats.

Treatment is started in a hospital setting so heart rhythm can be monitored closely. Antiarrhythmic drugs, including mexiletine, have not been shown to improve overall survival in patients with ventricular arrhythmias.
