Why this applies to Penicillamine

Penicillamine falls under ORAL DRUGS — the group Blond Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.

Here's the simple version: blond psyllium is a thick, gel-forming fiber that can grab onto medications in your gut and slow or reduce how much your body absorbs. With penicillamine, the concern is that the fiber gel could bind some of the drug, so less of it gets into your bloodstream. That matters because penicillamine works by latching onto excess copper (in Wilson's disease) or cystine (for kidney stones), and a weaker dose could mean it does its job less well.

The good news is this is a minor, mostly theoretical interaction, and it's easy to manage with timing. A common approach is to space them apart so the fiber and the medicine aren't sitting together. Penicillamine is already fussy about food and timing, so please check with your pharmacist or doctor before adding psyllium.

Blond psyllium is a soluble fiber from the seed husks of Plantago ovata, and it is one of the better-studied fiber supplements. It can help with both constipation and diarrhea and may modestly lower cholesterol and blood sugar. It is generally well tolerated when taken with plenty of water, but always start low and check with your pharmacist or doctor first.

What Is Penicillamine Used For?

Brand names: Cuprimine, Depen Titratable

Penicillamine is used for three distinct conditions. It treats Wilson's disease — a rare genetic disorder in which the body accumulates dangerous levels of copper — by binding to the excess copper and helping the body remove it. It also treats cystinuria, a condition where the amino acid cystine builds up and forms painful kidney stones, by converting cystine into a more soluble form that the body can excrete more easily.

For patients with severe, active rheumatoid arthritis who have not responded adequately to conventional therapies, penicillamine (available as Aagylur, Cuprimine, Depen, and other products) may be used to help suppress disease activity and reduce joint pain, tenderness, and swelling.
