Theoretically, taking boldo with hepatotoxic drugs might increase the risk of hepatic injury and disease.
Boldo leaf contains ascaridole, a known liver toxin. Many cases of hepatotoxicity, including elevated liver transaminase levels and jaundice, have been reported in patients taking boldo.

Here's the thing to keep in mind with bosentan (Tracleer): it can already be hard on the liver. That's why your doctor checks your liver enzymes with blood tests before you start and regularly while you take it. So adding anything else that might stress the liver is worth a careful look.

Boldo leaf contains a compound called ascaridole that has been linked to liver injury, including raised liver enzymes and jaundice (yellowing of the skin or eyes). Stacking it on top of bosentan could, in theory, raise your chance of liver problems. I'll be honest, this is based mostly on scattered case reports, not strong studies, so it isn't a sure thing. Still, with a liver-sensitive drug like this, I'd play it safe. Please check with your pharmacist or doctor before trying boldo, and don't change your bosentan on your own.

Boldo is a South American shrub whose leaves are traditionally used for digestive and gallbladder complaints, but good human evidence is very limited. The leaves and oil contain ascaridole, which can be toxic to the liver, so only ascaridole-free products should be used and only for short periods. Talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
