Bosentan falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the gist for bosentan (Tracleer). This medicine relaxes blood vessels in your lungs to ease pulmonary arterial hypertension, and a known part of how it works is gently lowering blood pressure. Goldenseal contains berberine, which in animal studies and some human research (mostly studied on its own, not as goldenseal) may also nudge blood pressure down.

So the theory is that stacking the two could add up and drop your pressure a bit more than expected. You might feel dizzy, lightheaded, or unusually tired, especially when standing up quickly. I'll be honest: this is based on pharmacology, not solid proof with goldenseal itself, so it's a low-level concern. Still, before adding goldenseal, run it by your pharmacist or doctor first.

Bosentan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP2C9 to help break down bosentan. Goldenseal, mostly because of its berberine and hydrastine, may gently block that enzyme. If it does, bosentan could clear a little slower and build up in your blood, which can mean stronger effects and a higher chance of side effects like headache, flushing, swelling in your legs or ankles, or feeling more tired. Bosentan can also stress the liver, so anything that raises its levels is worth watching closely.

The honest part: this concern comes only from lab studies, not from real people, so it's theoretical at this point. Still, since bosentan is a serious medication, please run goldenseal by your pharmacist or doctor before combining the two.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Bosentan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist for Tracleer (bosentan). Your body relies on a liver enzyme called CYP3A4 to break this medicine down. Goldenseal appears to slow that enzyme down, so bosentan can hang around longer than usual and build up in your blood. That's not how bosentan works in reverse; it's an active drug, not a prodrug, so more of it simply means stronger effects.

In real terms, that could mean more headache, flushing, swelling in your legs or ankles, or low blood pressure (feeling dizzy or lightheaded). Bosentan can also stress the liver, so anything that raises its levels deserves caution. The evidence here is moderate, not ironclad, but it's worth a quick chat. Please don't start or stop anything on your own; check with your pharmacist or doctor first.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
