Why this applies to Bedaquiline
Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body clears bedaquiline using a liver enzyme called CYP3A4. Boswellia, in lab studies, appears to slow that same enzyme down (and at very low amounts might even speed it up a little). If it slows things, bedaquiline could build up in your blood, which matters because one of its known concerns is an effect on heart rhythm (a change called QT prolongation) that other TB drugs in your regimen can also cause.
The honest part: this is theoretical, based only on test-tube data, not real patient reports, so we don't truly know if it happens in people. Still, MDR-TB treatment is too important to risk. Please run Boswellia past your TB doctor or pharmacist before adding it.

What Is Bedaquiline Used For?
Brand name: Sirturo
Sirturo (bedaquiline) is used to treat pulmonary tuberculosis (TB that affects the lungs) caused by bacteria resistant to at least two key TB drugs, rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).
Sirturo is always used in combination with other TB medications — never on its own. It is not for latent TB (TB infection without active symptoms), drug-sensitive TB, TB outside the lungs, or infections caused by other types of mycobacteria.
