Here's the gist for fedratinib (Inrebic): your body partly clears this medicine using a liver enzyme called CYP2C19. In lab studies, the boswellic acids in Boswellia appear to slow that same enzyme down. If that happens in real life, fedratinib could clear a bit more slowly and build up in your system, which may mean stronger effects and more side effects like nausea, diarrhea, vomiting, fatigue, or changes in your blood counts and liver tests.

I want to be honest about how soft this is. This is a theoretical concern based only on test-tube data, not on people taking both together. Fedratinib is also a serious cancer medicine that's monitored closely, so please don't add Boswellia on your own. Run it past your oncologist or pharmacist first.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the gist with fedratinib (Inrebic). Your body relies on a liver enzyme called CYP3A4 to help clear this medication out. Lab studies hint that boswellia (the resin in many joint-support supplements) may block that enzyme, and at very low amounts it might mildly speed it up instead. If boswellia slows that enzyme down, fedratinib could build up in your system, which may mean more of its known side effects, like nausea, diarrhea, fatigue, low blood counts, or changes in liver tests.

I want to be honest that this is theoretical, based on test-tube work, not real patient cases. Still, fedratinib is a serious cancer drug, so don't add boswellia without first checking with your oncology team or pharmacist.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
