Why this applies to Irinotecan Hydrochloride
Irinotecan Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with irinotecan (Onivyde) and boswellia. Your liver uses an enzyme called CYP3A4 to help process irinotecan. In test tube studies, boswellic acids seem to slow this enzyme down (and at low amounts may slightly speed it up). If boswellia blocks that enzyme in real life, more irinotecan could linger in your body, which may worsen its already tough side effects like severe diarrhea, low blood counts, nausea, and fatigue.
The honest truth is this is theoretical, based on lab work, not real patients. But irinotecan is potent chemotherapy with little room for error, so I wouldn't take chances. Please run boswellia by your oncology team before using it during treatment.
