Palbociclib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the gist for Ibrance (palbociclib). Your body relies on a liver enzyme called CYP3A4 to break this medication down. In lab studies, boswellia appears to slow that same enzyme (and at very low levels it might do the opposite). If boswellia blocks CYP3A4 in you, palbociclib could clear more slowly and build up, which may mean more of its usual side effects like low white blood cell counts, fatigue, mouth sores, or infection risk.

I want to be honest: this is theoretical and based on test-tube work, not real patient cases. Still, because palbociclib is a cancer drug where dosing matters, please don't add boswellia without first running it by your oncologist or pharmacist.

Theoretically, Boswellia serrata might alter the effects of immunosuppressive drugs.
Some in vitro research suggests that Boswellia serrata extracts might inhibit mediators of autoimmune disorders such as leukotrienes and reduce production of antibodies and cell-mediated immunity. However, other in vitro research suggests that, when coupled with calcium ions, boswellic acids containing the keto group have immunostimulant properties within specific cell signaling pathways.

I want to be upfront with you: the concern here is very theoretical. Boswellia has been grouped with this drug because lab and animal studies hint it might nudge the immune system in one direction or another. Palbociclib (Ibrance) isn't a typical immunosuppressant though. It's a targeted cancer drug that works by blocking a cell growth signal, and one of its main effects is already lowering your white blood cell counts.

So in real life, we don't have solid evidence that boswellia changes how palbociclib works or worsens its side effects like low blood counts, fatigue, or infection risk. Still, because this is a serious cancer treatment, please loop in your oncologist or pharmacist before adding boswellia so they can keep an eye on things.

About Palbociclib

What Is Palbociclib Used For?

Brand name: Ibrance

Palbociclib (brand name Ibrance) is used to treat adults with hormone receptor (HR)-positive, HER2-negative advanced or metastatic breast cancer. It is always given in combination with other medicines — either an aromatase inhibitor as a first hormonal treatment, or with fulvestrant after the cancer has progressed on prior hormonal therapy. It is also used with inavolisib and fulvestrant for a specific subtype (PIK3CA-mutated, endocrine-resistant) of this cancer confirmed by an FDA-authorized test.

Ibrance is also approved in combination with trastuzumab (with or without pertuzumab) and endocrine therapy for maintenance treatment of adults with HR-positive, HER2-positive locally advanced or metastatic breast cancer, following initial induction treatment.
