Rifabutin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the gist for rifabutin (Mycobutin). Your body relies on a liver enzyme called CYP3A4 to help clear this antibiotic out. In test-tube studies, boswellia seems to mostly slow that enzyme down (and at low amounts may slightly speed it up), so on paper it could nudge your rifabutin levels up or, less likely, down.

If levels crept up, you might notice more of rifabutin's usual side effects, like nausea, an upset stomach, or a harmless orange-pink color to your urine, tears, or sweat. The good news: this is purely theoretical, based on lab work, not real patients, so I wouldn't panic. Still, since you're treating a serious infection, just give your pharmacist or doctor a heads-up before adding boswellia.

What Is Rifabutin Used For?

Brand name: Mycobutin

Rifabutin capsules are used to prevent disseminated MAC (Mycobacterium avium complex) disease — a serious bacterial infection that can spread throughout the body — in patients with advanced HIV infection.

It is not approved to treat active tuberculosis (TB), and it must not be used alone in patients who have active TB, as this can lead to antibiotic resistance.
