Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Your body clears riociguat (Adempas) partly through a liver enzyme called CYP3A4. In lab studies, boswellic acids from Boswellia seem to slow that enzyme down (and at low amounts may slightly speed it up). If boswellia did block CYP3A4 in real life, riociguat could build up and hit harder. Since this medicine relaxes blood vessels, that mainly means a bigger drop in blood pressure, so watch for dizziness, lightheadedness, headache, or feeling faint when you stand up.

Here's the reassuring part: this concern comes only from test-tube research, not from people, so it's theoretical. Still, riociguat is a sensitive drug with careful dosing, so loop in your pharmacist or doctor before combining the two.

What Is Riociguat Used For?

Brand name: Adempas

Riociguat — sold as Adempas and under the generic name Riociguat — is used to treat adults with two conditions that cause dangerously high blood pressure in the arteries of the lungs.

  • Chronic Thromboembolic Pulmonary Hypertension (CTEPH): Used after surgical treatment, or when surgery is not an option, to improve exercise capacity and functional class.
  • Pulmonary Arterial Hypertension (PAH): Used to improve exercise capacity, functional class, and to delay the condition from getting worse. It can be used alone or combined with certain other PAH medicines.
