Selinexor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the gist for Selinexor (Xpovio). Your body uses a liver enzyme called CYP3A4 to help clear this medicine, and lab studies hint that boswellia can interfere with that same enzyme. Mostly it seems to slow CYP3A4 down, which could theoretically let more Selinexor build up. With a strong cancer drug like this, higher levels could mean rougher side effects, things like nausea, vomiting, fatigue, low appetite, low blood counts, or low sodium.

I want to be honest though: this concern comes only from test-tube research, not from real patients, so it's theoretical. Still, since Selinexor is a serious chemotherapy agent, please don't add boswellia without first running it by your oncology team or pharmacist.

What Is Selinexor Used For?

Brand name: Xpovio

Xpovio (selinexor) is used to treat multiple myeloma in adults. When used together with bortezomib and dexamethasone, it is for adults who have had at least one prior treatment. This combination is sometimes called XVd.

Xpovio is also used with dexamethasone alone (called Xd) for adults with relapsed or refractory multiple myeloma — meaning the cancer came back or stopped responding — who have already tried at least four prior therapies and whose disease no longer responds to several other standard drug types.
