Vandetanib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the short version on combining boswellia with vandetanib (Caprelsa). Vandetanib is partly cleared by a liver enzyme called CYP3A4, and it works as-is (it's not a prodrug that needs activating). In lab dishes, boswellia mostly slows CYP3A4 down. If that happened in the body, vandetanib could build up a bit, which might mean more of its side effects, like diarrhea, rash, high blood pressure, or changes in your heart's QT rhythm.

The big caveat: this is theoretical, based only on test-tube data, not real people, so we honestly don't know if it matters in practice. Since vandetanib is a serious cancer drug with real heart risks, don't start or stop anything on your own. Please run boswellia past your oncologist or pharmacist first.

Caprelsa (vandetanib) is used to treat medullary thyroid cancer — a type of thyroid cancer — in adults whose tumor cannot be surgically removed or has spread (locally advanced or metastatic disease) and is causing symptoms or actively growing.

Caprelsa should be used with extra caution in patients whose cancer is slow-growing and not causing symptoms, because the serious risks of the medicine must be carefully weighed against any potential benefit in that setting.
