Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the concern with pairing these two. Your body relies on a liver enzyme called CYP3A4 to break down acalabrutinib (Calquence). In lab studies, Bulbine natalensis appears to speed up that enzyme. If that happened in people, your body could clear acalabrutinib faster than intended, leaving less of it in your bloodstream. Since this is a cancer medicine, weaker drug levels are a real worry because the medication may not control your mantle cell lymphoma, CLL, or SLL as well as it should.

I want to be honest though: this is theoretical and comes only from test-tube work, not human studies. Still, with a cancer treatment, I wouldn't gamble. Please run any supplement past your oncologist or pharmacist before adding it.

Here's the situation with Calquence (acalabrutinib) and Bulbine natalensis. Acalabrutinib is moved around your body partly by a transporter called P-glycoprotein, which acts like a pump that helps escort the drug out. In lab dish studies only, Bulbine natalensis appeared to increase the gene signal for this pump. If that translated into more pump activity in people, it could theoretically push acalabrutinib out faster and lower its levels, which might mean less cancer-fighting benefit over time.

I want to be honest: this is very early, test-tube-level evidence, not something proven in humans, so I wouldn't panic. Still, with a cancer therapy this matters. Bulbine also raises liver and kidney safety concerns, so please check with your oncologist or pharmacist before combining them.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
