Acetaminophen, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bulbine Natalensis interacts with.
In vitro research shows that Bulbine natalensis stem extract slightly induces CYP1A2 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP1A2 substrates; however, this has not been studied in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: this is a multi-symptom cold and flu combo (a pain reliever/fever reducer, a cough suppressant, and a decongestant). The concern on file is that Bulbine natalensis may gently speed up a liver enzyme called CYP1A2. In theory, a faster enzyme could clear certain medicines a little quicker, which might make them work a touch less.
Honestly, though, this is only a lab/test-tube finding (no human studies), and these particular cold ingredients don't lean heavily on CYP1A2 anyway, so I wouldn't expect a meaningful problem from a typical short course. What does concern me more is that Bulbine itself has raised liver and kidney worries in animals, and acetaminophen is hard on the liver. I'd run it by your pharmacist before combining them.

In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo cold-and-flu product contains three actives, and the one most relevant here is dextromethorphan (your cough suppressant), which the body partly clears using the CYP3A4 enzyme. In lab and animal studies, Bulbine natalensis seemed to speed up that enzyme. If that held true in people, it could lower dextromethorphan levels and make your cough medicine feel a bit weaker. The acetaminophen and pseudoephedrine pieces aren't strongly tied to this enzyme, so they're less of a concern.
The honest truth is this is purely theoretical, based on test-tube and rodent data, not real patients, so I wouldn't lose sleep over it. I'm more cautious about Bulbine itself, since animal studies hint at possible liver and kidney strain. Please run it by your pharmacist or doctor before combining them.
