Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the concern with pairing this supplement and Alpelisib (Piqray or Vijoice). Your body clears alpelisib partly through a liver enzyme called CYP3A4, and lab studies suggest Bulbine natalensis may rev that enzyme up. If it speeds up clearance, your body could break down alpelisib faster, leaving less of it to do its job. For a cancer or overgrowth medicine like this, weaker drug levels could mean the treatment doesn't work as well as it should, which is a serious tradeoff.

I want to be honest that this is theoretical. It comes from test-tube research, not human studies, so we truly don't know how much it matters in real life. Given how important alpelisib is, please don't add this supplement without first checking with your oncologist or pharmacist.

About Alpelisib

What Is Alpelisib Used For?

Brand name: Piqray

Alpelisib is approved for two distinct uses, each tied to a specific brand. Piqray is used in combination with another drug called fulvestrant to treat adults with hormone receptor (HR)-positive, HER2-negative, advanced or metastatic breast cancer whose tumors carry a PIK3CA mutation — and only after the cancer has progressed on or after a hormone-based treatment. A companion diagnostic test is required before starting Piqray.

Vijoice is used to treat adults and children 2 years of age and older who have severe manifestations of PIK3CA-Related Overgrowth Spectrum (PROS) — a rare condition where PIK3CA gene mutations cause abnormal tissue, vascular, and bone overgrowth — and who need systemic (whole-body) therapy. Vijoice's approval for PROS was granted under the FDA's accelerated approval pathway based on response data, and continued approval may depend on confirmatory trials.
