Bepridil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the gist for Bepridil, an older heart rhythm and chest pain (angina) medication. Your body clears bepridil using a liver enzyme called CYP3A4. Lab studies suggest Bulbine natalensis may speed that enzyme up. If that happened in a person, your body could break down bepridil faster, leaving less of it in your bloodstream and possibly weakening how well it controls your angina or rhythm.

I want to be honest about the evidence: this is only a theory from test-tube and animal work, never confirmed in people, so the real risk is uncertain. Still, bepridil is a drug where blood levels matter, so don't add this supplement without checking first. Please talk with your pharmacist or doctor before combining them.
