Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the concern with pairing Bulbine natalensis with your HIV medicines, darunavir and ritonavir. Both of these rely on the liver enzyme CYP3A4, and lab studies suggest this plant may speed that enzyme up. If it does, your body could clear these drugs faster, leaving lower levels in your blood. With HIV therapy, weaker drug levels are a real problem because the virus can escape control and even develop resistance, which can jeopardize your whole regimen.

I want to be honest that this comes only from test-tube research, not human studies, so we don't truly know how strong the effect is. But because keeping HIV medicine levels steady is so important, I'd play it safe and talk with your doctor or pharmacist before adding this supplement.

Here's the honest picture with your HIV combo, darunavir plus ritonavir. Both are handled in part by a cellular pump called P-glycoprotein, which moves drugs in and out of cells. The interaction on file says Bulbine natalensis might affect that pump, but I want to be clear: this comes only from lab (test tube) research, not from people, so it's theoretical.

The lab work actually hints the plant could speed up that pump, which in theory might lower your drug levels a bit. With HIV medicine, keeping levels steady really matters, since dips can let the virus slip through. Add in the animal reports of possible liver or kidney strain, and I'd hold off. Please check with your HIV pharmacist or doctor before trying it.
