CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Dolutegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with Juluca (dolutegravir plus rilpivirine), which is a two-drug combo that keeps HIV under control. The rilpivirine part is cleared by a liver enzyme called CYP3A4. In lab studies, Bulbine natalensis seems to speed up that enzyme. If that happened in your body, your system could break down rilpivirine faster, lowering its blood levels.
Why that matters: with HIV medicine, even a modest drop in drug levels can let the virus rebound and possibly become resistant, which is a big deal. The good news is this is only a theoretical, test-tube finding, and it has never been confirmed in people. Still, I wouldn't risk it. Please check with your HIV provider or pharmacist before combining the two.

P-GLYCOPROTEIN SUBSTRATES

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Juluca (dolutegravir and rilpivirine), your HIV combo pill. One of these drugs, dolutegravir, is handled in part by a transporter called P-glycoprotein, which acts like a gatekeeper moving the drug in and out of your cells. In lab (test tube) studies, Bulbine natalensis appeared to turn up the gene that makes this transporter. If that holds true in people, it could push more of the drug out and possibly lower how much stays in your blood.
Why that matters: with HIV medicine, even small drops in drug levels can let the virus rebound or build resistance, so this isn't one to gamble on. That said, this is only a theoretical, test tube finding, not proven in humans. Please loop in your pharmacist or doctor before adding Bulbine natalensis, especially given its possible liver concerns.
