In vitro research shows that Bulbine natalensis stem extract slightly induces CYP1A2 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP1A2 substrates; however, this has not been studied in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on combining Bulbine natalensis with your leukemia medication, enasidenib (Idhifa). Lab studies hint that this plant might slightly speed up an enzyme called CYP1A2, which is one of the pathways your body uses to clear certain drugs. If that happened with enasidenib, your body could break it down a little faster, which in theory might lower its blood levels and make it work less well against the cancer cells.
I want to be straight with you though: this is only a theory from test-tube research, never shown in real people, so we don't actually know if it matters. Given that this is a serious cancer drug, please don't add any new supplement without first checking with your oncologist or pharmacist.

Here's the gist for Idhifa (enasidenib), your leukemia medicine. In lab dishes, Bulbine natalensis seems to rev up an enzyme called CYP2C9, which is one of the pathways your body uses to clear certain drugs. If that revving happened in real life, your body could break down and clear enasidenib a bit faster, which in theory might leave less of it working against the cancer cells.
I want to be honest about how shaky this is. This comes only from test-tube research, not from people, so we truly don't know if it matters at all in the body. But with a cancer drug, even a small loss of effect isn't worth guessing about. Please loop in your oncology team or pharmacist before adding this supplement.

Here's the concern with pairing this supplement and your leukemia medicine. Enasidenib (Idhifa) is handled in part by a liver enzyme called CYP3A4. In lab dishes, Bulbine natalensis appears to rev up that enzyme, meaning your body could clear enasidenib faster than intended. With a cancer drug, lower levels could mean it works less well at controlling the AML, which is the opposite of what you want.
I want to be honest though: this is only a theoretical worry based on test-tube data, not studies in people, so we can't say how much it really matters. Given that this drug is fighting leukemia and the supplement also raises liver concerns, please loop in your oncologist or pharmacist before adding it.

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
