Esterified Estrogens, Methyltestosterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Estratest combines an estrogen with methyltestosterone, and your liver uses an enzyme called CYP3A4 to help clear these hormones. Lab research suggests Bulbine natalensis may speed up that enzyme. If that happened in people, your body could break down the hormones faster, which might make Estratest work less well. In practice that could mean a return of symptoms it was prescribed for, like hot flashes, night sweats, or low libido.

I want to be honest about how thin the evidence is here. This is based only on test-tube and animal data, not on real patients, so it's theoretical. There are also separate concerns about Bulbine possibly stressing the liver and kidneys. Before combining the two, please check with your pharmacist or doctor.

Estratest already contains a testosterone-type hormone, methyltestosterone, paired with estrogens. Bulbine natalensis is sold as a natural testosterone booster, and in animal studies it appeared to nudge testosterone levels upward. So the worry here is simple: stacking a possible testosterone booster on top of a product that already delivers a testosterone medicine could add up and push the hormone effects too far.

If that happened, you might notice more of the usual androgen side effects, such as acne, oily skin, unwanted hair growth, mood changes, or fluid retention. I'll be honest, this concern is theoretical and based on animal data, not human studies, so we can't say how strong it really is. Given the liver concerns with Bulbine too, please check with your doctor or pharmacist before combining them.
