Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the situation with Fosaprepitant (Emend). After it's given through an IV, your body quickly converts it into aprepitant, the active form that helps prevent the nausea and vomiting that comes with chemotherapy. That active drug is broken down by a liver enzyme called CYP3A4. In lab and animal studies, Bulbine natalensis appears to speed up that enzyme, which could clear the drug out faster and leave less of it working.

In plain terms, the theoretical worry is a weaker anti-nausea effect, meaning chemo-related nausea or vomiting might not be controlled as well. This is only lab-based, not proven in people, so don't panic. Still, since this matters most during cancer treatment, please mention the supplement to your oncology team or pharmacist before combining them.
