Fostamatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the thing to know about pairing these two. Your body activates fostamatinib into its working form, called R406, and the liver enzyme CYP3A4 is heavily involved in handling it. Lab studies suggest Bulbine natalensis may speed up that enzyme. In theory, a faster enzyme could clear the drug more quickly and leave you with less of its effect, meaning your platelet counts might not stay where your doctor wants them. That could show up as easier bruising or bleeding.

I want to be honest about the evidence, though. This is purely test-tube and animal data, with nothing studied in real people, so it's a theoretical concern rather than a proven problem. Since fostamatinib treats a serious bleeding condition, please run any supplement plans past your pharmacist or doctor first.
