Ibuprofen, Hydrocodone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with this combo pill, which pairs ibuprofen (an anti-inflammatory) with hydrocodone (an opioid painkiller). In lab dishes, Bulbine natalensis appears to speed up a liver enzyme called CYP3A4, the same enzyme that helps process hydrocodone. If that sped-up effect happened in real people, your body might clear the hydrocodone a bit faster, which could mean weaker pain relief than you expect.
Here's the honest part: this is only a theory from test-tube work, never confirmed in humans, so I wouldn't lose sleep over it. Still, since this supplement also raises liver and kidney concerns and the evidence behind it is thin, please run it by your pharmacist or doctor before combining the two.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Your combo pill contains ibuprofen, which is an NSAID that already nudges your platelets toward being a little less sticky and can irritate the stomach lining. In lab (test tube) studies, Bulbine natalensis also slowed platelets from clumping together. Stack the two and, in theory, you could bruise more easily or bleed a bit longer from cuts. The bigger concern with ibuprofen specifically is the stomach, so watch for signs like black or tarry stools, vomit that looks like coffee grounds, or unusual stomach pain.
Keep in mind this is theoretical, based only on lab data, not real human cases, so don't panic. Still, given Bulbine's possible liver and kidney concerns, I'd run this past your pharmacist or doctor before combining them.
