Indacaterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Indacaterol is a long-acting inhaler you breathe in once a day to keep your airways open if you have COPD. Part of how your body clears it relies on a liver enzyme called CYP3A4. Lab studies suggest Bulbine natalensis may speed that enzyme up, which in theory could clear indacaterol a little faster and make it work less well, possibly leaving you more short of breath or wheezy than usual.

Here's the reassuring part: this is only a theoretical concern based on test-tube research, not anything proven in people. Indacaterol is also inhaled, so much of it acts right in the lungs. Still, since this supplement carries its own liver and kidney safety questions, please check with your pharmacist or doctor before combining the two.

Here's the quick picture for Indacaterol (Arcapta Neohaler, Onbrez Breezhaler), your long-acting inhaler for COPD. It's a substrate of a transporter called P-glycoprotein, which acts like a pump moving the drug around and out of your body. The note on file says Bulbine natalensis might affect that pump and could raise indacaterol levels in theory.

I want to be honest with you: this is very weak evidence. It comes only from lab-dish research, and that study actually pointed toward Bulbine speeding the pump up, which would more likely lower indacaterol, not raise it. The truth is nobody really knows. If levels did climb, you might notice more jitteriness, a fast or pounding heartbeat, or shakiness. Given Bulbine's own liver and kidney concerns, please check with your pharmacist or doctor before combining them.
