Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Bulbine Natalensis interacts with.
In vitro research shows that Bulbine natalensis stem extract induces CYP2C9 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP2C9 substrates; however, this has not been studied in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Opsynvi combines two medicines, macitentan and tadalafil, that work together to relax blood vessels and ease the strain of pulmonary hypertension. The concern here is theoretical: in lab and test-tube studies, Bulbine natalensis appears to speed up an enzyme called CYP2C9, which helps clear these drugs from your body. If that happens in people, your body might break down the medicine a little faster, which could leave less of it working and possibly weaken the benefit you count on.
I want to be honest that this has never been tested in humans, so it may not matter at all. Still, since this drug keeps a serious condition in check, please don't start Bulbine on your own. Check with your pharmacist or doctor first, especially given the liver worries with this supplement.

If the same thing happened in people, your body might clear this medication a little faster, which could make it less effective at controlling your pulmonary hypertension. That matters because this drug is working to ease the strain on your heart and lungs.
Here is the honest part: this is only a theory from test-tube research, not anything shown in humans, so I would not panic. Still, given how important this medicine is and the safety questions around this supplement, please check with your pharmacist or doctor before combining them.
