Nelfinavir Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing Bulbine natalensis with your HIV medicine, Viracept (nelfinavir). Your body clears nelfinavir using a liver enzyme called CYP3A4. In lab dish studies, this plant seemed to speed up that enzyme, which could make your body break down nelfinavir faster. That would mean lower drug levels in your blood. With an HIV medicine, that's a real worry, because too little drug can let the virus rebound and may even allow it to become resistant to treatment.
I want to be honest though: this is only a theoretical concern from test-tube work, not anything proven in people. Still, HIV therapy is too important to gamble with. Please check with your doctor or pharmacist before adding this supplement.

Here's the situation with Bulbine natalensis and your HIV medicine nelfinavir (Viracept). Nelfinavir is carried around your body partly by a transporter protein called P-glycoprotein, which acts a bit like a pump. In a lab dish, this plant appeared to ramp up the gene for that pump. If that also happened in people, the pump could push more nelfinavir out, lowering its levels in your blood.
For an HIV medicine, that's the real worry: too little drug can mean the virus isn't fully controlled and resistance can build. But honestly, this is only a theoretical concern from test-tube work, not proven in humans. Since keeping nelfinavir steady matters so much, please check with your pharmacist or doctor before adding this supplement.
