Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that Bulbine natalensis stem extract slightly induces CYP1A2 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP1A2 substrates; however, this has not been studied in humans.

Here's the short version for Zofran (ondansetron): your body partly clears it using a liver enzyme called CYP1A2. Lab studies suggest Bulbine natalensis may gently speed up that enzyme. In theory, that could mean your body breaks ondansetron down a little faster, leaving slightly less in your system. If that happened, the medicine might not control your nausea and vomiting as well as you'd expect.

I want to be honest about how thin this evidence is. This comes only from test-tube and animal research, not from people, and the effect described is small. Still, I'd skip Bulbine while you're relying on Zofran, partly because this supplement also raises liver and kidney concerns. Please check with your pharmacist or doctor before combining them.

Here's the idea behind this pairing. Your body uses an enzyme called CYP3A4 to help clear ondansetron (Zofran). In lab dish studies, Bulbine natalensis appeared to rev up that enzyme. If that happened in people, it could mean your body breaks down ondansetron a bit faster, leaving less of it to do its job. In plain terms, your anti-nausea coverage might not work as well, so you could still feel queasy or vomit even after taking it.

About Ondansetron

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
