Here's the honest picture for Opicapone (Ongentys). This interaction is only theoretical and comes from lab dish research, not from people. The concern is that Bulbine natalensis might nudge a transporter called p-glycoprotein, which can affect how some medicines move in and out of cells. The description on file suggests it could raise drug levels, though the lab data on this point is honestly mixed and incomplete.

In practice, we have very little solid information on how Bulbine would actually affect opicapone in your body. If levels shifted upward, you could in theory see more of opicapone's known effects, like extra involuntary movements (dyskinesia), low blood pressure, or sleepiness. Because the evidence is so thin and Bulbine itself raises liver and kidney concerns, please check with your pharmacist or doctor before combining them.

Ongentys (opicapone) is used alongside levodopa/carbidopa to treat Parkinson's disease in adults who experience 'off' episodes — periods when medication wears off and Parkinson's symptoms return before the next dose is due.

It is not used alone. Ongentys works as an add-on to help levodopa stay active longer, reducing the time patients spend in these 'off' states.
