Osilodrostat Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the basic idea with Isturisa (osilodrostat), which you likely take to lower the high cortisol of Cushing's disease. Lab studies suggest Bulbine natalensis may speed up an enzyme called CYP3A4, one of the pathways your body uses to clear this medicine. If that happens in people, osilodrostat could be broken down a little faster, which in theory might soften how well it controls your cortisol.

I want to be honest: this is only a theoretical concern from test-tube and animal work, not from human studies, so we really don't know how much it matters. Since osilodrostat is carefully dosed based on your cortisol levels, even small shifts are worth flagging. Please don't change anything on your own. Just check with your pharmacist or doctor first.
