Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the short version for Nurtec (rimegepant). Your body clears rimegepant using a liver enzyme called CYP3A4. In lab dishes, Bulbine natalensis seems to rev up that enzyme. If the same thing happened in people, your body could break rimegepant down faster, leaving less of it around. In real life that might mean your migraine medicine simply doesn't work as well, so a headache could come back or not respond like it usually does.

I want to be honest about how shaky this is. This concern comes only from test-tube studies, not from people, so we can't say it truly happens. Still, Bulbine itself raises liver and kidney worries, so please run it by your pharmacist or doctor before combining the two.

Here's the situation with Nurtec ODT (rimegepant) and Bulbine natalensis. Rimegepant is carried out of your body partly by a transporter called P-glycoprotein. It is an active drug, not a prodrug, so anything that changes that transporter can change how much rimegepant stays in your system.

The database note says Bulbine theoretically raises P-glycoprotein substrate levels, which could mean a bit more rimegepant hanging around and slightly stronger effects like nausea or stomach upset. But honestly, this is weak evidence from lab and animal studies only, and one test even pointed the opposite way (more transporter activity, which would lower levels). The real-world risk is very uncertain. Given that Bulbine itself raises liver and kidney concerns, I'd run it by your pharmacist or doctor first.
