Here's the thing with tamoxifen: your body actually has to convert it into its active form (called endoxifen) before it can do its job protecting against breast cancer. Enzymes in your liver, including CYP2C9, are part of that process. Lab studies suggest Bulbine natalensis may speed up CYP2C9, which in theory could change how much active drug you end up with. Because tamoxifen is a medicine you really want working at full strength, any unpredictable shift is a concern.

I want to be honest though: this is only a theoretical worry based on test-tube research, not anything proven in people. Still, with a cancer-prevention drug, I wouldn't gamble. Please don't start or stop anything on your own. Loop in your oncologist or pharmacist first so they can weigh in.

Here's the wrinkle with this one: tamoxifen is a prodrug. Your body has to convert it into its active form (mainly something called endoxifen) before it can do its job of fighting and preventing breast cancer. Several liver enzymes help with that activation, and CYP3A4 is one of them.

In lab dishes, Bulbine natalensis seems to speed up CYP3A4. The worry is that revving up this enzyme could shift how tamoxifen is processed and possibly lower the active levels that protect you, which matters a lot for a cancer medicine. I want to be honest though: this is only theoretical, based on test-tube data, with nothing studied in real people. Still, given the stakes (and that this supplement has liver safety concerns of its own), please run it by your oncologist or pharmacist before combining them.

Here's the short version on pairing Bulbine natalensis with tamoxifen. Tamoxifen rides through your body partly on a transporter pump called P-glycoprotein, which helps move the drug in and out of cells. In a lab dish, Bulbine appeared to turn up the activity of that pump. If that held true in people, it could push more tamoxifen out, which might mean a little less of it sticking around to do its job.

I want to be honest with you, though: this is purely test-tube, theoretical data, and the actual effect on the live transporter was never measured. So I wouldn't lose sleep over it. That said, tamoxifen is too important to gamble with, and Bulbine itself has animal-based liver and kidney concerns. Please run it past your oncologist or pharmacist before adding it.

About Tamoxifen

What Is Tamoxifen Used For?

Brand names: Nolvadex, Soltamox

Tamoxifen is used to treat metastatic breast cancer (breast cancer that has spread) in both women and men, and as adjuvant (after-surgery) therapy for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.

In women with ductal carcinoma in situ (DCIS) — a very early, non-invasive form of breast cancer — tamoxifen is used after breast surgery and radiation to reduce the risk of developing invasive breast cancer. It is also approved to reduce the incidence of breast cancer in women who are at high risk but have not yet been diagnosed. Soltamox (oral solution) is specifically indicated for all of these uses in adult patients.
