Some animal research suggests that cabbage or its constituent indole-3-carbinol might increase drug metabolism and elimination by stimulating CYP1A2 activity.

Here's the bottom line on eating cabbage alongside this cold-and-flu combo (which packs acetaminophen, chlorpheniramine, and dextromethorphan). The concern on file is that compounds in cabbage might speed up a liver enzyme called CYP1A2. If that enzyme runs faster, it could in theory clear a medication out of your body a little quicker, which might make it work less well. None of these three ingredients are prodrugs, so faster breakdown would mean slightly weaker effect, not stronger.
Honestly, this is very theoretical. It's based only on lab and animal studies, and these specific drugs don't lean heavily on CYP1A2 anyway. Normal amounts of cabbage as food are not something to worry about here. If you're taking concentrated cabbage or indole-3-carbinol supplements, just mention it to your pharmacist.

Here's the gist for this multi-symptom cold product. The acetaminophen (the pain and fever reducer) is partly cleared by a process called glucuronidation, where your body tags the drug so it can be flushed out. Cruciferous veggies like cabbage may slightly speed that process up, which could lower acetaminophen levels a bit and make the fever and aches relief feel weaker. The dextromethorphan (cough) and chlorpheniramine (antihistamine) parts aren't the main concern here.
Honestly, this is mostly theoretical and based on small studies using daily, heavy cabbage intake. A normal serving of cabbage with dinner is very unlikely to matter. If you eat large amounts regularly and feel your cold medicine isn't working as well, just check in with your pharmacist.

This combo product (Vicks Formula 44M) contains acetaminophen for aches and fever, plus chlorpheniramine and dextromethorphan for sniffles and cough. The piece that interacts with cabbage is the acetaminophen. Cruciferous veggies like cabbage and Brussels sprouts can gently speed up how your liver processes acetaminophen (through a pathway called glucuronidation), so a little less of it stays in your system. One small study saw acetaminophen levels drop by about 16%.
In plain terms, eating a lot of cabbage might make the acetaminophen part work slightly less well for fever or pain. This is a modest effect, and normal food amounts usually aren't a big deal. The cough and antihistamine ingredients aren't affected. If you rely on this for fever and it doesn't seem to help, check with your pharmacist or doctor.
