Why this applies to Grepafloxacin
Grepafloxacin falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cabbage interacts with.
Some animal research suggests that cabbage or its constituent indole-3-carbinol might increase drug metabolism and elimination by stimulating CYP1A2 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for grepafloxacin (Raxar): this antibiotic is partly cleared by a liver enzyme called CYP1A2, and cabbage may gently nudge that enzyme to work a little faster. If that happened, your body could clear grepafloxacin a bit quicker, which in theory might leave slightly less drug around to fight your infection. Grepafloxacin is an active antibiotic, not a prodrug, so faster breakdown means potentially weaker effect, not stronger.
Honestly though, this concern comes only from lab and animal studies, so the real-world impact in people is uncertain and likely small. Normal food amounts of cabbage in your meals shouldn't be a problem. If you're eating very large amounts or taking concentrated cabbage or indole-3-carbinol supplements, just check with your pharmacist or doctor first.
