Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Cannabinol (cbn) interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of cannabinol.
In vitro research shows that CYP3A4 enzymes are involved in the 8-hydroxylation of cannabinol. So far, this interaction has not been reported in humans.

Here's the gist for this pairing. Fosaprepitant (Emend) is the IV version of aprepitant, a medicine given to head off the nausea and vomiting that come with chemotherapy. One thing it does in the body is slow down a liver enzyme called CYP3A4. As it happens, that same enzyme helps break down CBN, so if you slow it down, CBN may hang around longer and feel a bit stronger than expected.

In plain terms, that could mean more of CBN's usual effects, like extra drowsiness or grogginess. I'll be honest though, this is only based on lab studies and hasn't actually been seen in people, so it's more theory than proven worry. Still, since Emend is given around chemo, it's worth running CBN by your oncology team or pharmacist first.
