Brivaracetam falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Cannabis interacts with.

Cannabis may increase levels of drugs metabolized by CYP2C19.
Research shows that cannabidiol (CBD), a constituent of cannabis, inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabis with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the short version for Briviact (brivaracetam): your body partly clears it using a liver enzyme called CYP2C19. The CBD in cannabis can slow that enzyme down. When the enzyme works slower, brivaracetam may not get broken down as quickly, so its levels in your blood can drift upward. Brivaracetam is not a prodrug, so more drug hanging around means stronger effects, including possible drowsiness, dizziness, fatigue, trouble with coordination, or feeling foggy.

That said, this is a theoretical concern based on how these compounds behave, not solid proof for this exact pair. Occasional or food-level exposure is less likely to matter than regular, concentrated CBD products. Please don't stop or change anything on your own. Just loop in your doctor or pharmacist so they can watch for added side effects.

What Is Brivaracetam Used For?

Brand name: Briviact

Brivaracetam — available as tablets, oral solution, and injection under the brand name Briviact, as well as generic forms — is indicated for the treatment of partial-onset seizures (seizures that begin in one part of the brain) in patients 1 month of age and older. It can be used alone (monotherapy) or alongside other seizure medicines (adjunctive therapy).

Briviact injection is specifically intended for intravenous use when a patient is temporarily unable to take medicine by mouth — it is not meant for long-term IV use. The oral tablet and oral solution forms are used for ongoing treatment.
