Here's the thing to know if you're using cannabis while taking Akynzeo (netupitant and palonosetron). Netupitant gently blocks a liver enzyme called CYP3A4, and that same enzyme helps break down THC, the part of cannabis that gets you high. So in theory, your body may clear THC a little more slowly, letting it build up and hit harder than usual.

In practice, that could mean stronger cannabis effects like extra drowsiness, dizziness, a racing heart, or feeling more impaired. This is a theoretical concern based on how the drugs work, not solid human studies, so I don't want to alarm you. Still, since Akynzeo is given during chemo when you already feel rough, it's worth mentioning your cannabis use to your oncologist or pharmacist.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist for Akynzeo (netupitant plus palonosetron). The netupitant part is cleared from your body mainly by a liver enzyme called CYP3A4. Lab studies suggest cannabis compounds (especially CBD and CBG) can mildly slow this enzyme down. If that happens in real life, netupitant could hang around a bit longer and build up slightly, which might mean more of its usual side effects, like headache, constipation, fatigue, or feeling weak.

That said, this is based only on test-tube research, not real patient cases, so the risk is theoretical and likely modest. Since Akynzeo is given to keep chemo nausea under control, you don't want surprises. Please mention any cannabis use to your oncology team or pharmacist so they can keep an eye on things. Don't change anything on your own.

What Is Netupitant and Palonosetron Used For?

Brand name: Akynzeo

Netupitant and palonosetron helps prevent nausea and vomiting from cancer chemotherapy in adults. It is used together with dexamethasone, a steroid medicine. It covers both the first day (acute phase) and the days after (delayed phase).

Akynzeo capsules are used for initial and repeat courses of chemotherapy, including highly emetogenic (strongly nausea-causing) chemotherapy. Akynzeo for injection and Akynzeo injection are used for highly emetogenic chemotherapy only. They have not been studied with anthracycline plus cyclophosphamide chemotherapy.
