Aluminum Hydroxide, Aspirin, Codeine Phosphate, Magnesium Hydroxide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Clove interacts with.

Theoretically, concomitant use of clove may increase levels of drugs metabolized by CYP2D6.
In vitro research shows that eugenol, the principal constituent of clove, can inhibit CYP2D6 in a dose-dependent manner. This effect has not been reported in humans.

Here's the piece that matters with this combo product: the codeine part is a prodrug. Your body has to use an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. Lab studies (not human ones) suggest eugenol, the main compound in clove, can slow that same enzyme down.

So in theory, if clove blocks CYP2D6, less codeine gets converted, meaning you might get less pain relief, not more. The good news is this is only test-tube evidence, and normal cooking amounts of clove almost certainly won't matter. Concentrated clove oil or capsules are where I'd be more cautious. If you lean on this medicine for real pain, check with your pharmacist before using clove supplements.

Theoretically, clove oil may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Laboratory research suggests that eugenol, a constituent of clove, has antiplatelet activity. This interaction has not been reported in humans.

This combination product already contains aspirin, which is itself a blood thinner that keeps your platelets from clumping together. The thing to know about clove is that one of its compounds, eugenol, has shown some platelet-blocking activity in lab studies. So in theory, layering concentrated clove oil or clove supplements on top of the aspirin here could nudge your bleeding risk a little higher than the aspirin alone.

Keep this in perspective. This is a theoretical concern based on test-tube work, and it hasn't actually been reported in people. The sprinkle of clove you'd use in cooking is very unlikely to matter. If you're using strong clove oil or supplements regularly, just give your pharmacist or doctor a heads-up, and watch for easy bruising, nosebleeds, or bleeding gums.
