Theoretically, combining coleus with antihypertensive drugs might cause additive blood pressure lowering effects and increase the risk of hypotension.

Here's the simple version. Macitentan (Opsumit) treats the high lung pressure of PAH by relaxing and widening blood vessels, which also nudges your overall blood pressure down. Coleus contains forskolin, and in animal studies that compound may lower blood pressure too. So stacking the two could, in theory, drop your pressure more than either alone.

If that happened, you might feel lightheaded, dizzy, faint, or unusually tired, especially when standing up quickly. I want to be honest though: this is a theoretical concern based mostly on lab and animal data, not strong human studies. Please don't start or stop anything on your own. Just give your pharmacist or doctor a heads-up before adding Coleus so they can keep an eye on your blood pressure.

Macitentan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, taking coleus might decrease serum levels of drugs metabolized by CYP3A4.
In vitro research shows that coleus can activate the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of coleus and other drugs affected by these enzymes.

Here's the gist for Opsumit (macitentan) and Coleus. Your body relies on a liver enzyme called CYP3A4 to break down macitentan. Lab studies suggest the forskolin in Coleus may rev up that enzyme, so in theory your body could clear macitentan a little faster. That could mean less drug working, which matters because macitentan's whole job is keeping the blood pressure in your lungs under control.

I want to be honest though: this is based only on test-tube research, not real patients, so we don't truly know if it happens in people. Since macitentan is so important for your PAH, please check with your pharmacist or doctor before adding Coleus rather than changing anything on your own.

Coleus is a plant from the mint family whose root contains a compound called forskolin, often marketed for weight loss, asthma, and heart health. While early lab and small human studies are interesting, solid proof that Coleus supplements help any of these conditions is limited. What Is Macitentan Used For?

Brand name: Opsumit

Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a serious condition where the blood pressure in the arteries supplying the lungs is abnormally high. The goal is to slow down how quickly the disease gets worse and to lower the chances of being hospitalized because of it.

In clinical studies, it was shown to be effective in adults with mostly moderate-to-severe PAH symptoms. Patients in those studies had PAH caused by unknown or inherited reasons, connective tissue disorders, or congenital heart disease with repaired heart shunts.
