Glipizide, Metformin falls under ANTIDIABETES DRUGS — the group Colocynth interacts with.

Theoretically, colocynth might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Small clinical studies show that colocynth fruit powder reduces fasting blood glucose levels in patients with type 2 diabetes who are also taking antidiabetes drugs.

Here's the thing to watch with Metaglip (glipizide plus metformin): both medicines already work to lower your blood sugar. Some small studies suggest colocynth can push your fasting blood sugar down too. Stack the two together and you may drop too low, which is called hypoglycemia. With glipizide especially, that can mean shakiness, sweating, a racing heart, confusion, or feeling faint.

The evidence here is fairly limited, so this is more of a real possibility than a sure thing. But honestly, colocynth is also quite toxic and a harsh laxative, so most pharmacists, myself included, would steer you away from it. Please don't add it to your diabetes routine without first checking with your doctor or pharmacist.
