Phenobarbital, Sodium Nitrate falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Coltsfoot interacts with.

Hepatotoxic pyrrolizidine alkaloids (PA) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the concern with pairing coltsfoot and phenobarbital. Coltsfoot can contain pyrrolizidine alkaloids, plant compounds that are already known to be hard on the liver. Phenobarbital revs up a liver enzyme called CYP3A4, and in theory that faster activity could turn more of those alkaloids into the harmful, liver-damaging form. So this isn't about your phenobarbital working differently. It's about your liver potentially taking a bigger hit from the herb.

I'll be honest, this idea comes from lab and animal research, not real-world human studies, so it's more of a caution than a proven danger. Still, coltsfoot's liver risks are reason enough to be careful. Please check with your pharmacist or doctor before using it, and lean toward products certified free of these alkaloids.
