Metformin, Sitagliptin falls under ANTIDIABETES DRUGS — the group Coriander interacts with.

Theoretically, coriander might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Evidence from animal research suggests that coriander fruit and coriander extract can reduce blood glucose levels. Monitor blood glucose levels closely. Dose adjustments might be necessary.

Here's the simple version: Janumet combines two diabetes medicines, metformin and sitagliptin, and both work to lower your blood sugar. Coriander, in animal studies, also seems to nudge blood sugar down a bit. Put them together and the worry is that your numbers could dip a little lower than expected.

If that happens, you might feel shaky, sweaty, dizzy, or unusually hungry, which are the classic signs of low blood sugar. Honestly though, this concern comes from lab and animal research, not solid human studies, and the cilantro you sprinkle on tacos is very unlikely to do much. Concentrated coriander extracts or oils are where I'd pay closer attention. If you use those, keep an eye on your glucose readings and check in with your pharmacist or doctor before changing anything.
