Efavirenz falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Ergot interacts with.

Ergotamine, a constituent of ergot, is a substrate of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that inhibit CYP3A4 might increase the risk of ergot toxicity. Some of these drugs include amiodarone (Cordarone), clarithromycin (Biaxin), diltiazem (Cardizem), erythromycin (E-mycin, Erythrocin), indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase), and many others.

Here's the gist for you. Ergot's main active chemical, ergotamine, gets cleared out of your body by a liver enzyme called CYP3A4. Some medicines slow that enzyme down, which can let ergot pile up and trigger ergot toxicity (think severe headache, cold or tingling hands and feet from squeezed-down blood vessels, nausea, even dangerous circulation problems). Efavirenz can have a mild blocking effect on this enzyme, so in theory it could nudge ergot levels higher.

I want to be honest about the strength here. This is based on weak, anecdotal evidence, and it's labeled moderate. The bigger point is that raw ergot is genuinely toxic and shouldn't be taken as a supplement at all. Please don't change your HIV medicine, and talk with your pharmacist or doctor before using any ergot product.

What is Ergot?

Ergot is a toxic fungus that grows on rye and contains powerful chemicals (ergot alkaloids) that strongly narrow blood vessels and squeeze the uterus. Because raw ergot is poisonous and the dose is impossible to control safely, it should never be used as a home remedy or supplement. Some purified ergot chemicals are used as prescription drugs under strict medical supervision, but the crude herb is not safe to take.

What Is Efavirenz Used For?

Brand name: Sustiva

Efavirenz is used to treat HIV-1 (human immunodeficiency virus type 1) infection. It is always used together with other antiretroviral medicines — never on its own.

It is approved for adults and for children who are at least 3 months old and weigh at least 3.5 kg (about 7.7 lbs). Both the capsule and tablet forms are approved for this use.
