Here's the concern with pairing St. John's wort and efavirenz (Sustiva). This herb revs up a liver enzyme called CYP3A4, which is one of the pathways your body uses to clear efavirenz. When that enzyme speeds up, your body burns through the medication faster, and the level in your blood can drop.

With an HIV medicine, that's a serious problem. Lower efavirenz levels mean the drug may not fully control the virus, and that can lead to treatment failure and even resistance, where the medication stops working for good. This isn't a small side-effect issue, it's about keeping your HIV suppressed. Because of this, St. John's wort is generally considered off-limits with efavirenz. Please don't start or stop anything on your own, talk with your HIV provider or pharmacist first.

Efavirenz also falls under NON-NUCLEOSIDE REVERSE TRANSCRIPTASE INHIBITORS (NNRTIs).

St. John's wort decreases the levels and clinical effects of NNRTIs.
St. John's wort increases the oral clearance of nevirapine (Viramune) by 35%. Subtherapeutic concentrations are associated with therapeutic failure, development of viral resistance, and development of drug class resistance. St. John's wort induces intestinal and hepatic cytochrome P450 3A4 (CYP3A4) and intestinal P-glycoprotein/MDR-1, a drug transporter.

Here's the concern with this pairing. St. John's wort revs up certain liver and gut systems (CYP3A4 and a transporter called P-glycoprotein) that your body uses to process efavirenz. When those systems speed up, they clear efavirenz out of your blood faster, so the levels can drop too low to keep HIV in check.

Efavirenz is not a prodrug, so this works in the direction you'd worry about: less drug, less effect. The real danger isn't a side effect, it's losing control of the virus and risking resistance, which can make your whole regimen harder to treat. This is a well-documented, serious interaction. Please don't start or stop anything on your own. Talk with your HIV provider or pharmacist first so they can keep your treatment working.

What Is Efavirenz Used For?

Brand name: Sustiva

Efavirenz is used to treat HIV-1 (human immunodeficiency virus type 1) infection. It is always used together with other antiretroviral medicines — never on its own.

It is approved for adults and for children who are at least 3 months old and weigh at least 3.5 kg (about 7.7 lbs). Both the capsule and tablet forms are approved for this use.
