Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the thing to know about this pairing. Adrafinil only works after your liver converts it into modafinil, and that modafinil is then broken down by an enzyme called CYP3A4. Efavirenz can influence that same enzyme, so on paper it could shift how much active modafinil sticks around. If levels run higher, you might notice more of the stimulant effects, like trouble sleeping, jitteriness, a racing heart, headache, or feeling anxious.

I'll be honest with you though, this is a theoretical concern based on how the drugs work, not on real-world reports, so it may not amount to much. Efavirenz itself is also a complicated drug for your HIV treatment, and adrafinil is unregulated and can stress the liver. Please loop in your doctor or pharmacist before combining them.

Here's the gist for efavirenz (Sustiva): when you take adrafinil, your liver turns it into modafinil, and modafinil can mildly speed up an enzyme called CYP3A4. Efavirenz is partly broken down by that same enzyme, so in theory revving it up could lower your efavirenz levels a bit.

Why that matters with an HIV medicine: keeping drug levels steady is important for keeping the virus controlled and avoiding resistance. I want to be honest though, this concern is theoretical and based on how the drugs work, not on real patient reports, so the effect is likely small. Still, with HIV treatment I'd rather be cautious. Please run this by your doctor or pharmacist before combining them.

What Is Efavirenz Used For?

Brand name: Sustiva

Efavirenz is used to treat HIV-1 (human immunodeficiency virus type 1) infection. It is always used together with other antiretroviral medicines — never on its own.

It is approved for adults and for children who are at least 3 months old and weigh at least 3.5 kg (about 7.7 lbs). Both the capsule and tablet forms are approved for this use.
