Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.

Here's the honest version: this combination pill (acetaminophen, butalbital, and codeine) isn't a strong match for the CYP1A2 interaction on file. Evodia is thought to speed up an enzyme called CYP1A2, but the main ingredients here lean on different liver pathways. Codeine, for example, is a prodrug that relies mostly on CYP2D6 to turn into its active form, and butalbital and acetaminophen are handled through other routes.

So in plain terms, I wouldn't expect Evodia to dramatically change how well this medicine works for your headaches. And keep in mind, the evidence comes only from lab and animal studies, not people. Still, because this product contains a barbiturate and an opioid, please run any Evodia use past your pharmacist or doctor first.

Theoretically, evodia might reduce the levels and clinical effects of CYP2E1 substrates through induction of CYP2E1.
Animal research suggests that rutaecarpine, a constituent of evodia, induces CYP2E1 activity. In rats, rutaecarpine increases markers of CYP2E1 activity, and administration of rutaecarpine with chlorzoxazone, a known CYP2E1 substrate, reduces the area under the curve (AUC) of chlorzoxazone by 84% and increases its clearance by 646%.

Here's the wrinkle with this combo. Evodia (specifically a compound called rutaecarpine) may speed up a liver enzyme known as CYP2E1. The acetaminophen part of Phrenilin #3 is partly handled by that same enzyme. In theory, revving up CYP2E1 could shift more acetaminophen toward a toxic byproduct called NAPQI, which is the chemical behind acetaminophen-related liver stress. So the worry here isn't a weaker headache pill, it's a possible added burden on the liver, especially with regular use or alcohol.

I want to be honest about the strength of this: it's based only on animal and lab data, so it's more of a theoretical caution than a proven problem. Still, please run it by your pharmacist or doctor before pairing them, and avoid going over the daily acetaminophen limit.
