Evodia and Diphenhydramine Hydrochloride, Naproxen Sodium

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Evodia interacts with whole categories of medication. Diphenhydramine Hydrochloride, Naproxen Sodium falls into 2 of those categories, so it can interact with Evodia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Diphenhydramine Hydrochloride, Naproxen Sodium

Diphenhydramine Hydrochloride, Naproxen Sodium falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Evodia interacts with.

Evodia might reduce the levels and clinical effects of CYP1A2 substrates through induction of CYP1A2.
Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Aleve PM combines naproxen (a pain and inflammation reliever) with diphenhydramine (the sleep-aid antihistamine, the same ingredient as Benadryl). The concern with Evodia is that it may speed up a liver enzyme called CYP1A2. If that enzyme runs faster, it could clear out drugs that rely on it a bit quicker, which might mean slightly weaker effects from this combo product.

Honestly, this is a theoretical worry. The evidence comes only from test tube and animal studies, and neither naproxen nor diphenhydramine leans heavily on CYP1A2, so a big change is unlikely. You might just notice your pain relief or drowsiness feels a touch less reliable. If you use Evodia regularly, mention it to your pharmacist or doctor so they can keep an eye on things.

ANTICOAGULANT/ANTIPLATELET DRUGS

2 of 2

Diphenhydramine Hydrochloride, Naproxen Sodium also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, taking evodia with antiplatelet or anticoagulant drugs might increase the risk of bruising and bleeding.
In vitro and animal studies show that rutaecarpine, a constituent of evodia, inhibits platelet aggregation.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind with Aleve PM: the naproxen part already nudges up your bleeding and bruising risk on its own, since it's an NSAID that affects how platelets stick together and can irritate the stomach lining. Evodia contains a compound called rutaecarpine that, in lab and animal studies only, seems to slow platelet clumping too. So theoretically, pairing the two could add up and make easy bruising, nosebleeds, or stomach bleeding a bit more likely.

I want to be honest though, this is very early evidence and we don't have real human data showing it happens. The diphenhydramine (the sleep ingredient) isn't the concern here. Still, since naproxen already carries some bleeding risk, I'd run this combo by your pharmacist or doctor before adding evodia, and watch for unusual bruising or dark stools.
