Fosamprenavir falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Evodia interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the gist of this one: fosamprenavir (Lexiva) blocks an enzyme called CYP3A4, which is one of the main pathways your liver uses to break down evodia's active compounds. So when you take them together, the worry isn't really about your HIV medicine changing. Instead, evodia itself may not get cleared as quickly, so its levels could climb higher than usual and stick around longer.

I'll be straight with you: this is a theoretical concern. It comes from animal and lab studies using a different blocker (ketoconazole), not from people taking fosamprenavir. We don't fully know what extra evodia in the system would feel like. Since your HIV regimen is too important to risk, please run this by your pharmacist or doctor before adding evodia.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist with Evodia and your HIV medicine fosamprenavir. Fosamprenavir is actually a prodrug. Once you swallow it, your body turns it into amprenavir, the form that fights the virus. Amprenavir is then cleared by a liver enzyme called CYP3A4. In test-tube studies, evodia appears to slow that same enzyme down.

If that effect happened in people, the active drug could build up a bit and possibly bring more side effects, things like nausea, diarrhea, rash, or headache. But honestly, this is only based on lab research, not on real patients, so it's theoretical for now. To be safe, run any evodia supplement past your pharmacist or HIV provider before adding it.

What Is Fosamprenavir Used For?

Brand names: Lexiva, Telzir

Fosamprenavir tablets (sold as Fosamprenavir Calcium and Lexiva) are used together with other antiretroviral medicines to treat HIV-1 infection in adults and in children as young as 4 weeks old. It is not used alone — it is always part of a combination regimen.

For adults who have already taken protease inhibitor medicines before, fosamprenavir is used alongside a low dose of ritonavir. Once-daily dosing with ritonavir is not recommended for adults who have previously used protease inhibitors, or for any pediatric patients.
