Why this applies to Ibrutinib

Ibrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Evodia interacts with.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for Ibrutinib (Imbruvica): your body relies on a liver enzyme called CYP3A4 to break this medicine down. Lab studies suggest evodia may slow that enzyme. If it does the same in people, ibrutinib could clear out more slowly and build up in your blood, which can mean stronger effects and more side effects.

With ibrutinib, that could look like easier bruising or bleeding, diarrhea, fatigue, infections, or heart rhythm changes. The important thing to know is that this concern comes from test tube research only, not from real patients, so we can't say for sure it happens in humans. Still, because ibrutinib is a serious cancer drug, please check with your oncologist or pharmacist before adding evodia.

Theoretically, taking evodia with antiplatelet or anticoagulant drugs might increase the risk of bruising and bleeding.
In vitro and animal studies show that rutaecarpine, a constituent of evodia, inhibits platelet aggregation.

Here's the thing to know if you take ibrutinib (Imbruvica) and are curious about evodia. Ibrutinib already carries its own bleeding risk on its own. It can lower how well your platelets stick together, so people on it may notice easier bruising, nosebleeds, or bleeding that takes longer to stop. Evodia contains a compound called rutaecarpine that, in lab and animal studies, also seems to make platelets less sticky.

Stack those two together and, in theory, you could see a bit more bruising or bleeding than either would cause alone. I'll be honest though: this is based on test tube and animal research, not human studies, so it's a possible concern rather than a proven one. Before adding evodia, please check with your oncologist or pharmacist first.

What Is Ibrutinib Used For?

Brand name: Imbruvica

Imbruvica (ibrutinib) is used in adults to treat chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL), including cases with a specific genetic change called 17p deletion. It is also used in adults to treat Waldenström's macroglobulinemia (WM), a rare type of slow-growing blood cancer.

Imbruvica is also approved for adults and children aged 1 year and older with chronic graft-versus-host disease (cGVHD) — a serious immune complication that can occur after a stem cell or bone marrow transplant — when at least one prior treatment has not worked.
